Looking at those I'm starting to think it might actually be cheaper to just get a new rack instead.
Wow. Sounds like a lot of unnecessary material to clamp across.
There are racks out there which will allow normal sized skewers - I was looking at this type last year eg. http://www.tredz.co.uk/.Axiom-Streamliner-Disc-Deluxe-Rear-Rack_54702.htm or if you don't have seatstay mounts either http://www.tredz.co.uk/.Axiom-Streamliner-Road-Deluxe-Rear-Rack_54703.htm which mounts at axle and behind a caliper brake. Tubus racks can be fitted like this with an optional adapter set but it makes an already expensive rack astronomical.
If you need standard length closed cam skewers but don't want to spend vast sums... Turbo trainer skewers can be had for just a few quid.
Planet X have 200mm fatbike skewers, but again they're open cam.

I've measured the skewer spindle, it is 230mm. And the parts of the rack where it goes through are at least an inch, yes.

230mm?!?
That just sounds wrong. It's nearly FOUR inches longer than a standard road or mtb rear skewer. Even fat bikes ain't that fat across the rear.
Try measuring between the clamping faces with everything done up on the bike. Unless there is something seriously rare about either your axle, dropouts or the rack struts are an inch thick I just can't see how you would need a skewer that size.
Fat bike hubs go up to 190mm - Hope do them, not internal cam but well made. Not currently listed anywhere but try a fat bike specialist like Sideways Cycles, Slam69 or Charlie the Bikemonger
