I know this may sound a bit odd but its something I found out the hard way after getting numb and hot feet.
Are you doing your shoes up to tight? As i said i used to get hot numb feet most rides and after looking about a bit and speaking with my doctor it was just doing my shoes to tight. What was happening is with the heat and not much movment my feet were swelling ever so slightly and was then restricted by my shoe causing the numbness, it may be worth a shot on the rollers just ratchet then 1 or 2 less than normal.
Maybe take the innersoles out and have a good feel about on the sole base of the shoe itself where the ball of you foot would be. See if you've ground a dent or hole in the sole.
My problem started wearing an old knackered'ish pair of sidi dominator mtb shoes, the innersoles seemed to sort it out straight away but after a while it started to creep back. Took em back to the shop and they had a closer look at the shoe and I'd rounded out a hole on the ball and under the big toe which the sole had eventually settled into.
They gave me a new set of soles with reinforced bits under the areas in question so they wouldn't sink in again, worked a treat and I can still wear the old shoes but they're even better when paired with a new set of shoes..
No worries, they're good guys. When you say fitted innersoles, which ones have you got?
I ask as I was getting a similar problem, couldn't work it out. Had a set of those specialized BG soles in but it turns out they were doing nothing. Seems I have super high arches, as they had no support my foot was rolling around on the ball and giving me jip. Was also getting tingling toes as a result as they were trying to grip hard to try and steady things up.
Cyclefit did their footbed moulding thing and sorted me out a pair of Sidas innersoles. They've been a revelation, feet feel great now, no twisting no nothing. I climb a little tiny bit better as a result too. I'll have say they're up there with new wheels n the like in my top cycling purchases.
Sidas moulded, like you. From Bespoke fwiw, but presumably the same outcome. They definitely improve things for a bit and generally improved the comfort overally.

I would get in touch with the guys at cyclefit in Covent Garden, they're pretty much as good as it gets in my eyes. They've also got a big network of specialists at their disposal too..
