GlaxoSmithKline run a loan bike scheme - I remember reading in one of the cycling mags that they are one of the more enlightened employers when it comes to cycling, even have a bike shop at head office.

We have a pool bike scheme in our office, as we were given a grant to buy some folding bikes.
I guess it'll be different from organisation to organisation, but we have 3rd party cover under the companies general insurance provision, and essentiall ride the bikes 'at our own risk' (Hi vis, helmets etc. are all available). All potential users were offered training (all declined) which I guess covers the company in terms of ensuring we're competent to use them.
Years ago I worked for United Utilities, who have to offices in the Warrington area, just under a mile apart. They had a 'fleet' of bikes for use between the offices that needed booking-out through reception.. I'm not sure if they still do, or even who you could chat with, but you could call : 01925 237 000 and ask.
